Glossary
Coinsurance
The amount you are required by your insurance company to pay after you have met your deductible. The coinsurance rate is generally expressed as a percentage. Example: If the insurance company pays 75% of the claim. You pay 15%.

Co-payment or Co-pay
Refers to the payment that the insured must pay at the time of an office visit..

Deductible
Is a predetermined dollar amount which the covered individual must pay  in each deductible accumulation period (generally 1 year) before any payment of benefits will be made by the insurance company.

Emergency Services
Any services provided within 48 hours of either an injury or a medical emergency.

Medical Emergency
The sudden onset of a medical condition with acute symptoms of severity such that the absence of immediate medical treatment could result in:

A) permanently placing in jeopardy, the health of the patient;
B) causing other possibly serious medical consequences;
C) causing serious impairment to bodily functions;
D) causing permanent and serious dysfunctions to any body part;

Medical Necessary
Describes health care services, treatments or supplies which are appropriate and consistent with the diagnosis and treatment of a medical condition according to generally accepted medical standards.

Out-of-pocket expenses
Refers to the amount a covered person will have to pay “Out of their own pocket” in a calendar year.

Pre-existing condition
Condition for which a person received medical services, treatment, care,  medication, diagnosis, or consultation 12 months prior to the covered person’s effective date of coverage or a condition that produced symptoms that are significant and distinct enough to establish the onset of a condition or that the condition manifested itself, where a person practiced in medicine would be able to diagnose the condition with reasonable certainty because of those symptoms.

Premium
The total amount you pay in exchange for insurance coverage.

Any person (Doctor, Nurse, Dentist) or institution (hospital or clinic) that provides medical care.
